Your release form
NOTE: This template is a starting point, not legal advice. Have it reviewed for your situation and local law before anyone signs it. VIDEO RELEASE FORM I agree that [Company name] may use the video I recorded, including my image, voice, name and words, in our website, our social media accounts and paid advertising on platforms such as Meta, TikTok and YouTube. Payment: I give this permission without payment. Withdrawal: I can withdraw this permission for future use at any time by writing to [Company name]. Release form or UGC contract?
They solve different problems. Use the one that matches who made the video.
| Document | Best for | What it covers |
|---|---|---|
| Release form | Customers and people appearing in your videos | Permission to use their image, voice and words |
| UGC contract | Creators you hire to make content | Deliverables, fee, usage rights, exclusivity |
| Both | A creator who also appears on camera | The contract, with a release clause inside it |
FAQ
Questions.
What is a video release form?
A short agreement where a person gives a business permission to use a video of them, including their image, voice and name, for stated purposes and a stated period.
Do I need a release form for customer testimonial videos?
It is good practice whenever you publish someone's likeness, and especially before using it in paid ads. A clear, signed release protects both you and the customer.
Can someone withdraw a video release?
That depends on what the form says and on local law. This generator lets you offer withdrawal for future use, which builds trust with customers.
Should I pay people who sign a release?
It is optional, but paying for ad use is fair and makes the permission clearer. The form records any payment you agree.
How long should a video release last?
Long enough for how you plan to use the video. 12 months is common for ads; website use is often longer. Avoid open-ended permission unless you really need it.
How does Trueclip handle release forms?
Customers who record through Trueclip choose whether to allow paid ads in the same flow, and their signed choice and license dates are stored with the video.
How do I make a release form?
State who is giving permission and to whom, what the recording is, every place it may be used, how long the permission lasts, any payment, and whether it can be withdrawn. Finish with name, date and signature lines. The generator above does all of this in plain English.
What if I don't have a model release form?
Using someone's likeness commercially without clear permission is risky and, in many places, not allowed, especially in ads. Get written permission before publishing, even from happy customers.
What do I need for a release form?
The person's name and signature, the date, your company's name, a description of the video, the permitted uses and duration, any payment, and a way to contact you.
What is the difference between a media release and a general release?
A media or video release gives permission to use someone's image, voice and words. A general release is a broader legal document where someone gives up claims against another party, for example after a settlement. They are not interchangeable.
Is a photo release the same as a video release?
They work the same way. Many businesses use one photo and video release form that covers both, as long as it names every use you plan.
